Feedback Forms
There are two class feedback forms available.

1) This Single Session Feedback form is similar to the one provided to students when they register for a class. It should be collected at the conclusion of your class and sent to LLU when you submit your class roster for payment. The student may mail in the feedback form.

2) The Multi-Session Feedback form is designed for classes that meet more than once or are an all day class. Collect the responses after the first class (first half for a full day class) and read your reviews, then send them to us when you submit your class roster for payment. If your feedback is less than what you want, hopefully the feedback will give you guidance.
